Joining the clinic
If you are interested in joining our practice as a new patient, we kindly ask that you complete the application form below and we will contact you as and when a vacancy become available.
Our New Patient Consultation, which includes all necessary dental imaging, charting, and treatment planning, costs £165. The purpose of this consultation is to give you options, where necessary, for achieving your dental fitness and most importantly keeping you there.

Visiting us
As primary health care providers, we have implemented the following operating procedures:
● We no longer have a waiting room – please let us know when you arrive, and then wait in your car, so that you can see the front door, until called. There is a clearly marked parking area for patients right outside.
● If you are attending with children, please come in to the clinic with one adult per child if at all possible.
● We do not have public toilet facilities.
● We accept payment by card, cheque or BACs. We don’t keep cash on the premises so if you wish to pay by cash you will need the correct money – we can’t give you change.
We must have a new medical form every year for every patient. This form needs to be received at least one working day before your appointment. We have made these forms available on our web portal, that you can access, complete and sign on a computer or mobile phone. We just need an email address and a mobile phone number for you, so if we don’t have that information, and you would like to use this service, please let us have:

If we do not have a current medical history we will not be able treat you.
If you are unable to complete an online form, please let us know and we will send you a paper version – again this needs to be received before your appointment – YOU CAN NOT BRING IT WITH YOU ON THE DAY
